The Lifelong Connections program helps to establish permanent connections with supportive adults for youth who are aging out of the foster care system. Staff work on identifying supportive adults and connecting them with youth aging out of Division of Child Permanency and Protection supervision. These relationships allow for adolescents and young adults to work towards goals of independent living, long-term specialized care, or stabilization.

Referrals can be made from any service provider or Division of Child Protection and Permanency office if the youth is under the supervision of the counties eligible.
