Lifelong Connections

The Lifelong Connections program helps to establish permanent connections with supportive adults for youth who are aging out of the foster care system. Staff work on identifying supportive adults and connecting them with youth aging out of Division of Child Permanency and Protection supervision. These relationships allow for adolescents and young adults to work towards goals of independent living, long-term specialized care, or stabilization.

Who Can Refer

Referrals are made through the Division of Child Protection and Permanency. Youth whose DCP&P Case is under the supervision of the above counties are eligible.
