Nurse-Family Partnership

The Nurse-Family Partnership Program is a free and voluntary service that pairs a first-time mom by 28 weeks gestation with a personal registered nurse to ensure the best possible pregnancy outcome, the healthy and appropriate growth and development of the baby, and to support the family in becoming self-sufficient. Moms can keep the service until their child’s 2nd birthday.

Approximately 2 ½ years

Who Can Refer

Anyone can refer. Referrals are commonly received by obstetrics providers, high schools, the Division of Child Protection and Permanency, and Women, Infants, and Children (WIC). Individuals can also self-refer.
